Washington, known as The Evergreen State, has a well-developed Medicaid Waiver and Home and Community-Based Services (HCBS) system designed to provide long-term care alternatives for seniors, individuals with disabilities, and those with chronic health conditions.

Administered by the Washington State Health Care Authority (HCA) and the Department of Social and Health Services (DSHS), Aging and Long-Term Support Administration (ALTSA), and the Developmental Disabilities Administration (DDA), the state’s Medicaid waiver programs allow eligible individuals to receive services in their homes or community settings rather than institutional care.
Key waiver programs include the Community Options Program Entry System (COPES) Waiver, which provides personal care, respite, and home modifications for seniors and adults with physical disabilities, and the Individual and Family Services (IFS) Waiver, which offers residential, employment, and behavioral health support for individuals with intellectual and developmental disabilities (IDD).
Other significant programs include the New Freedom Waiver, the Basic Plus Waiver, the Community Protection Waiver, and the Children’s Intensive In-Home Behavioral Support (CIIBS) Waiver, all of which provide specialized, person-centered services. Washington also offers Self-Directed Services, allowing waiver participants greater control over their care and choice of caregivers.
Regulatory oversight in Washington is managed through HCA, DSHS, ALTSA, and DDA, with additional provider licensing and compliance enforcement handled by the Residential Care Services (RCS) Division and the Office of the Medicaid Fraud Control Unit (MFCU). The state operates under both fee-for-service and managed care models, with HCBS services integrated through Apple Health Managed Care Organizations (MCOs) to enhance service delivery and cost-effectiveness. Washington has also implemented Electronic Visit Verification (EVV) for personal care services, ensuring compliance and service integrity. Providers must navigate Medicaid enrollment, waiver-specific service authorizations, and stringent quality assurance measures to participate successfully in the state’s HCBS system.
