Providers must ensure meals meet age-appropriate dietary requirements and accommodate special dietary needs while adhering to accessibility standards.

For HCBS services under 12VAC30-120-900, providers must document individualized service plans (ISPs), progress notes, and community integration activities that align with person-centered care principles. Documentation must reflect the individual’s preferences, goals, and outcomes while ensuring compliance with Medicaid waiver requirements for private duty nursing, consumer-directed care, and other waivered services. Providers are required to maintain records securely and retain them for specified periods to support audits and regulatory reviews.
