Providers are required to maintain comprehensive documentation, including individualized service plans (ISPs), progress notes, attendance logs, and quarterly reviews. Documentation must demonstrate how services support the individual’s goals for community integration, skill development, and social interaction. All records must be completed promptly—typically within 24 hours of service delivery—and securely stored in compliance with HIPAA standards. Community engagement services must occur in integrated community settings, not in residential or day program facilities, and adhere to group size limits of no more than three individuals per direct support professional.
